In order to setup your account with OrthoBanc, your orthodontist has disclosed to OrthoBanc certain nonpublic personal information about you. This information may include your name, address, social security number, telephone numbers, bank or credit card account numbers, and email address. We use this information to facilitate the transfer of funds from your bank or credit card account to your orthodontist to pay for their services. When you view your payment history, we may ask you to provide missing information such as work and home phone numbers or your e-mail address. We use this information if we need to contact you about your account.

We will not disclose any of the information we collect, as described above, to any party other than you or your orthobanc.
